Types of admissions.

Module 1- Intro and General discussion of the aging process
Topic: We will start this course talking about the general aging process our loved one goes through. We will explore the typical fears of our aging parents and typically how the body ages overall.
Worksheet: Prior Level of Function worksheet - This is a worksheet which you fill out to have on hand documenting how much your loved one can do for themselves. Why is this important? When your loved one ends up in the hospital, you have a sheet to copy and give to the therapist and social worker so they know what level of function your loved one needs to regain in order to return home.
Module 2- Hard discussion
Topic: This module will explore the hard discussion about POA vs DPOA, what an Advance Directive is and how it is different than a POLST, when to place a loved one in a care facility, and the money factor. We will also start the exploration of family dynamics and changing roles, but these two topics will be a part of all we talk about in the following modules.
Worksheet: Physician Orders for Life Sustaining Treatment (POLST)-a CMS (Medicare) official form used in every medical care facility and usually filled out by your loved one's primary care physician (PCP) or by any doctor when an acute event happens.
Module 3- Mindset
Topic: We will talking about caregiver burnout and how to create daily strategies for dealing with burnout. In this module, we will also be looking at problem vs solution-based thinking and the use of respite care to help with burnout.
Worksheet: Nourishing vs Depleting Worksheet-A worksheet to help identify task that nourish us vs those that deplete us and strategies for increasing our "nourishing" tasks.
Module 4-Medicare vs Medicare Advantage vs Medicaid
Topic: This module will be information heavy talking about the ins and outs of these three insurances as a payor source, and how they affect your loved ones access to care. We will explore the difference between Original Medicare and Medicare Advantage Plans, along with how Medicaid becomes a payor source. This is where we will talk about how or if you can become a paid caregiver for your loved one.
Worksheet: Compare and Contrast Worksheet-A worksheet to fill out so you have an easy reference when talking about the different payor sources.
Module 5-Common diagnoses and Events in the Elderly
Topic: This module will be information about the most common diagnoses which happen as we age and recurring medical events that happen. We will talk about what these diagnoses mean and how to prevent some of the common "things" that happen. For example, what to do when they aren’t eating as well as they used to or maybe completely stop eating, and why this even happens. Hint, it's about taste buds and sugar!
Worksheet: Note Taking Worksheet-A worksheet to fill out so you have an easy reference when talking about the different diagnoses and event your loved one is experiencing.
Module 6-8: Advocating for care in The Hospital, Rehab/SNF and Home health settings
Topic: This module will be an exploration about who pays for care, common expectations of the setting, understanding common diagnoses for admission, exploration of why you are filling out a POLST again, staff roles, what to look for in discharge planning, and loved ones fears about being in each of these settings.
While there are many similarities between what will be covered in this module, all lessons are site specific will educate on the similarities and differences in each individual setting.
Worksheet: Advocacy Vocabulary Worksheets-A worksheet to have on hand with setting specific vocabulary for advocating for your loved when they are in any of the above settings.
Module 9-Comfort care and hospice
Topic: This will be one of the hardest but most rewarding modules we discuss. This is where we will talk about the difference between palliative, comfort, and hospice care. Here is where we will explore what quality of days look like vs quantity of days. In this module we will also discuss who pays for these different services and why doctors always make these terms so confusing!
Worksheet: End-of-Life Discussion Workbook-A workbook to help you and your loved one talk about what end-of-life care will look like and how to decide quality of days over quantity of day.
